Welcome to online registration for Awana at United Baptist Church

There will be a registration fee per child for the 2022-2023 Awana year. 

1 Child = $25

2 Children = $45

3 Children = $60

4 Children = $80

This fee will go towards your child's book and any awards that they earn as they complete sections throughout the year. 

Fees are due ASAP. Visitors may attend up to 3 weeks only without registering/payment is due. Books will not be distributed until fees have been paid. 

All checks can be made out to United Baptist Church with a memo of "Awana Registration".

Please turn in any payments to Kara Felty or any AWANA worker at the sign in table. 

 

*Any bags, uniforms, and additonal books will be a seperate charge*

** Refunds are not eligible after 3 weeks of clubber attendance. 

**Refunds will not be given for handbooks that are written in or uniforms/vest/bags/other items that have been distributed.

Medical Release: As a parent/guardian, I give my permission for the above minor to attend Awana activities including regular club nights and any special activities from September 2022 to May 2023. I authorize treatment under the direction of any licensed physician of the above minor in the event of a medical emergency which in the opinion of the attending physician my endanger his or her life, cause disfigurement, physical impairment or undue discomfort if delayed. This authority is granted after reasonable effort has been made to reach me by phone. I will not hold the church, or their staff, administration, or workers, liable for any injury to or loss of possessions by the above minor during any activity either on the church property or away, including regular meetings as well as special events.
